Localities: DESMOND is currently being delivered throughout Brent. Brent Sites - Wembley and Willesden Centre for Health and Care
Local Referral Pathway: Referrals received from GP practice, internal referrals from other Health care professionals within the CLCH Healthcare NHS trust (i.e. Consultant, Nurses, Dietitians).
Modules Delivered:
We deliver the newly diagnosed and foundation DESMOND course to participants in the formats below:
Face-to-face 6hour full day session on Weekdays: Two full day sessions available weekly in Wembley and Willesden
Virtual program is split into two 3hour half day sessions delivered via Microsoft Teams on weekdays and weekends:
Program on Weekdays: Two a month (conducted over four 3hour half days)
Program on Saturdays: One a month ( conducted over two 3hour half days
