Localities: DESMOND is currently being delivered throughout Harrow

Local Referral Pathway: Referrals received from GP practice, internal referrals from other Health care professionals within the Harrow Diabetes Service i.e. Consultant, Diabetes Specialist Nurses.

Modules Delivered:

DESMOND Newly Diagnosed module available (for patients up to 12 months post diagnosis)

DESMOND Foundation module available (for those with established diabetes)

We deliver the Newly Diagnosed and Foundation DESMOND course to participants in the form of 2 sets of virtual sessions/month (split into 3 hours over 2 days via Microsoft Teams), 5 x Face-to-face 6-hour full day session/month (including 1 x Saturday session)

Overview

We are a team of Diabetes Specialist Nurses and Dietitians. We offer both clinical diabetes care and structured education for people with Type 2 diabetes.
